kels, the info you provided in our conversation was very valuable, thanks so much! also, i was able to find that missing form. i went through so much effort to find such a simple form (in comparison to the others). i found it by googling it! lol

Thanks for sharing those pics, JamCan. You guys all look like you had some much fun. Maybe hubby and I will join in on a future trip next year. I've never seen many areas of Jamaica the way I wanted to as a child, so I'm delighted for the resources available through ETJ. Folks like you, Samba, Tia and the like are certainly an asset to this site.
